4 Stages of Appendicitis: Symptoms, Diagnosis, Treatment

Posted on March 11, 2026 by Admin

Here’s a detailed overview of the 4 stages of appendicitis, including symptoms, diagnosis, and treatment options. 🩺

1. Early (Inflammatory) Stage

Symptoms:

  • Mild pain near the belly button (periumbilical)
  • Nausea, mild vomiting
  • Loss of appetite
  • Low-grade fever

Diagnosis:

  • Physical exam: tenderness in the lower right abdomen may not yet be obvious
  • Blood tests: slight elevation in white blood cells (WBC)
  • Imaging (ultrasound/CT) may show early inflammation

Treatment:

  • Often hospitalization and monitoring
  • Early surgical removal (appendectomy) may be recommended
  • Antibiotics can sometimes be used in very early or mild cases

2. Suppurative Stage (Pus Formation)

Symptoms:

  • Pain localizes to the lower right abdomen (McBurney’s point)
  • Increasing fever and nausea
  • Vomiting and abdominal tenderness
  • Possible mild diarrhea or constipation

Diagnosis:

  • Blood tests: higher WBC count indicating infection
  • Imaging: ultrasound or CT shows inflamed appendix with fluid accumulation

Treatment:

  • Appendectomy is standard (laparoscopic or open surgery)
  • Intravenous antibiotics before and after surgery

3. Gangrenous Stage

Symptoms:

  • Severe, continuous abdominal pain
  • High fever
  • Rapid heart rate, nausea, vomiting
  • Abdomen may become rigid and very tender

Diagnosis:

  • Blood tests: very high WBC, possible signs of systemic infection
  • Imaging: appendix shows necrosis (dead tissue)

Treatment:

  • Emergency surgery required
  • IV antibiotics essential to prevent infection spread
  • Delay in treatment increases risk of perforation

4. Perforated Stage (Ruptured Appendix)

Symptoms:

  • Sudden relief of pain followed by worsening generalized abdominal pain
  • Severe abdominal swelling (peritonitis)
  • High fever, chills, nausea, vomiting
  • Signs of shock in severe cases

Diagnosis:

  • Blood tests: infection markers elevated, possible sepsis indicators
  • Imaging: CT shows rupture, abscess, or fluid collection in abdomen

Treatment:

  • Emergency surgery to remove appendix and clean abdominal cavity
  • IV antibiotics for several days
  • Drainage of abscess if present

⚠️ Key Points

  • Appendicitis is a medical emergency; early diagnosis prevents complications.
  • Pain starting near the belly button that moves to the lower right abdomen is a classic early sign.
  • Do not attempt home remedies if appendicitis is suspected; seek urgent medical care.

If you want, I can also create a simple diagram showing all 4 stages with their key symptoms and treatments, making it easy to visualize progression and urgency.
